United Church Homes Skilled Nursing Communities Earn High CMS Ratings

MARION, Ohio — United Church Homes announced its skilled nursing communities have earned strong ratings in the latest Centers for Medicare & Medicaid Services Five-Star Quality Rating System.

The company reports that across its owned and managed communities, all United Church Homes skilled nursing communities received a 5-star Quality Measures rating, except The Four Winds Community in Jackson, Ohio, which received a 4-star Quality Measures rating, which is above-average performance and a strong indicator of high-quality care.

Owned communities receiving strong ratings include The Chapel Hill Community in Canal Fulton, Ohio; The Parkvue Community in Sandusky, Ohio; The Trinity Community at Fairborn in Fairborn, Ohio; The Trinity Community at Beavercreek in Dayton, Ohio; and The Four Winds Community in Jackson, Ohio.

Managed communities recognized include SEM Haven Health Center in Milford, Ohio; Wexner Heritage Village in Columbus, Ohio; Lutheran Home in Toledo, Ohio; and Lutheran Village at Wolfcreek in Holland, Ohio.

The CMS Five-Star Quality Rating System gives consumers a standardized way to compare skilled nursing residences across several performance categories. The Quality Measures category evaluates clinical and functional outcomes such as falls with injury, pressure ulcers, hospital readmissions, antipsychotic medication use and improvement in mobility and daily functioning.
