12 Oaks Senior Living Adds Sundance at WoodCreek Reserve to Its Houston-Area Communities

A unique program will soon begin supporting those living with memory loss in Katy, Texas. 12 Oaks Senior Living, a regional operator based in Dallas, has assumed management of Sundance at WoodCreek Reserve. The free-standing 40-unit memory care community will now be rebranded as WoodCreek Bend Memory Care by 12 Oaks Senior Living. Along with the new name, the community will begin implementing the Symphony of Life® program, which brings harmony to every movement and activity of community life and helps those suffering from memory loss stay engaged and active with everyday life.
“We are pleased to have been entrusted by an Agency REO Manager to turnaround some of their assets” said Lori Jones, RN, Chief Operating Officer. “We are excited to provide new direction and operational support to advance the quality of life for residents and associates. As part of our performance improvement plan for the community, we will be implementing 12 Oaks proprietary “Symphony of Life” programming to enhance the resident experience, which will also support residents’ ability to communicate with family members.”
The senior living management and consulting organization has more than 30 years of experience in providing senior living communities with highly effective and exceptional third-party management and consulting services. The management company oversees 11 communities throughout Texas and Oklahoma. The addition of WoodCreek Bend Memory Care by 12 Oaks Senior Living will make three in the greater Houston area.
