United Church Homes’ NaviGuide Earns NCQA Accreditation

MARION, Ohio — UCH Engage LLC, a subsidiary of United Church Homes, has earned a two-year National Committee for Quality Assurance Case Management for Long-Term Services and Supports accreditation for its NaviGuide program.
The accreditation is effective Aug. 18, 2026, through Aug. 18, 2028, according to a recent news release.
NaviGuide is a service coordination program that is designed to help older adults remain independent at home while navigating healthcare, community resources and daily living needs. The program offers subscription-based services and also participates in the Centers for Medicare & Medicaid Services’ GUIDE Model, or Guiding an Improved Dementia Experience.
UCH says participants are paired with a professional coordinator who can assist with healthcare needs, service coordination, caregiver support and connections to community resources. For people living with dementia and their caregivers, NaviGuide also provides care navigation, caregiver education and coaching, ongoing monitoring and connections to community-based resources.
“NaviGuide was created to bring peace of mind to older adults and their families by helping them navigate the often-complicated systems of healthcare, benefits and community services,” said Ken Young, CEO of United Church Homes.
NCQA Case Management Accreditation evaluates organizations that provide coordinated services and support to people with complex health and social needs. Its standards address areas including quality improvement, care coordination, person-centered support and management of long-term services and supports.
